What Is Outstanding Inside

Outstanding Inside begins with a question that every care home should be able to answer honestly: what is it actually like to live here?

Policies can describe good care. Training records can show that learning took place. Care plans can document preferences, risks and needs. Yet none of these, by itself, tells us how a person experiences an ordinary Tuesday afternoon. Does someone notice when she enters the room? Does he understand what is happening during personal care? Is choice offered in a way that still feels like choice? Does the environment help people recognise where they are, or quietly remind them of everything they can no longer do?

Outstanding Inside is a practical culture-change framework concerned with that space between what a service intends and what a person actually experiences. Its purpose is to make the less visible parts of care visible, then turn what we notice into practical change.

The question beneath the systems

Care homes need systems. Safe staffing, medicines management, governance, care planning and risk management matter enormously. Outstanding Inside does not compete with those responsibilities. It asks what happens when the system reaches the person.

A task may be completed safely and still leave someone feeling hurried. An activity may appear on the calendar while residents remain disconnected for most of the day. A newly decorated corridor may look attractive to visitors without helping the people who live there orientate themselves, recognise their surroundings or begin a conversation.

This is why our work focuses on lived experience. We look beyond whether something exists and ask what it enables. What does the dining room enable people to feel and do? What does the morning routine communicate about choice? What does staff language suggest about the way a resident is understood? What do leaders repeatedly notice, tolerate, praise and correct?

The Seven Human Needs

Outstanding Inside uses seven human needs as a practical lens: Being Seen, Feeling Safe, Having Choice, Being Understood, Feeling Valued, Staying Connected and Finding Meaning.

These are not presented as newly discovered psychological needs. They are a practice-based translation of established person-centred principles into language that staff can recognise in daily work. They help a team move from the broad instruction to be person-centred towards a more useful question: which human need is being met, missed or misunderstood in this moment?

Our practical method

The Outstanding Inside method moves through five stages: Notice, Understand, Design, Embed and Evidence.

We notice the resident experience rather than assuming it. We seek to understand what behaviour, language, routines and environments may be communicating. We design practical responses around real people and real homes. We embed those responses into ordinary work instead of relying on one enthusiastic individual. Finally, we gather evidence of what changed and how people experienced it.

The work is expressed through connected practice areas including Culture by Design™, Dementia Decoded™, Environment by Design™ and Power of Language™. Audits, observations, micro-learning and practical tools help translate the framework into action.

Built inside care

Outstanding Inside grew from work inside care homes, where the same gap appeared in different forms: good intentions, completed training and carefully written documentation could coexist with residents spending long stretches of the day unseen, unoccupied or disconnected.

Closing that gap does not always require a grand intervention. Often it begins with a member of staff introducing herself before providing care, waiting long enough for an answer, understanding distress as communication, offering a meaningful role, or using a corridor wall to invite recognition rather than merely filling space.

Outstanding care starts on the inside because culture is formed inside hundreds of repeated moments. When those moments change, the home begins to feel different from within.
